What the studies reported

Sepsis 28-day mortality meta-analysis

A meta-analysis of 11 randomized controlled trials (967 patients on thymosin alpha-1, 960 controls) reported a significant reduction in 28-day mortality overall (odds ratio 0.73, 95% CI 0.59 to 0.90, p = 0.003), but subgroup analyses restricted to high-quality trials (odds ratio 0.82, 95% CI 0.65 to 1.03, p = 0.09) and multicenter trials (odds ratio 0.86, 95% CI 0.68 to 1.08, p = 0.20) did not show a significant mortality benefit, and the authors noted the current evidence base is inadequately powered [1].

COVID-19 meta-analysis found no overall mortality benefit

A systematic review and meta-analysis pooling 9 studies (5352 patients: 1152 on thymosin alpha-1, 4200 controls), not all of which were randomized controlled trials, found no statistically significant effect on mortality overall (risk ratio 1.03, 95% CI 0.60 to 1.75, p = 0.92, I-squared 90 percent), and the authors concluded their results do not support use of thymosin alpha-1 in hospitalized adult COVID-19 patients; a mortality benefit appeared only in specific subgroups such as patients over age 60 and severe/critical cases [2].

What does dosing evidence actually support?

The cited sepsis meta-analysis pooled 11 randomized controlled trials (967 vs 960 patients) assessing 28-day mortality, and the COVID-19 meta-analysis pooled 9 studies (5352 patients total, not all randomized) assessing mortality; both used doses set by the original individual trial protocols in hospitalized patients, which are not reproduced here. These are pooled clinical trial results in specific hospitalized populations, reported as historical study findings only, not a general recommendation.

A study dose is not a personal dose. Population, route, formulation and supervision are part of the result. Online community schedules are not treated here as established clinical protocols.

Names are not interchangeable

Reign Peptide lists Thymosin Alpha-1 as a lyophilized powder (10mg/vial, greater than or equal to 99% purity by third-party HPLC per the listing), described by the supplier as for in-vitro research use only. This differs from the pharmaceutical-grade thymalfasin product used in the clinical trials summarized above, and no direct comparison between the two was identified in the sources reviewed.

Follow validated product instructions
