Regulatory status

Tesamorelin is FDA-approved under the brand EGRIFTA SV for reduction of excess abdominal fat in adults with a specific HIV-associated condition. Ipamorelin has no such approval. It is a synthetic pentapeptide described in the pharmacology literature as a selective growth hormone secretagogue, based on animal and in vitro potency data.

What the combination vial does not prove

A tesamorelin plus ipamorelin combination product exists on the market. No clinical or preclinical study of that specific combination was identified in the literature reviewed. Pairing an approved drug with an unapproved research peptide in one vial does not extend tesamorelin's approval to the mixture, and it does not give ipamorelin new evidence.

Selectivity vs approval

Ipamorelin's appeal in the literature is receptor selectivity at a potency similar to a reference peptide, which theoretically limits some side effects seen with older secretagogues. That is a mechanistic argument, not a substitute for the clinical trial data behind tesamorelin's approved indication.

Bottom line: Tesamorelin has an actual FDA approval, for a narrow indication and at a defined clinical dose, which ipamorelin does not have for any use. Treat the combination product as a supplier packaging decision, not a validated protocol.
